The Public Employee’s Benefits Board (PEBB) Program’s open enrollment begins Nov. 1 and runs through Nov. 30. All of the current medical and dental plans for employees will continue to be available in 2015.
New this open enrollment – If your spouse or registered domestic partner is enrolled in medical on your PEBB account for 2015 you must re-attest to the spousal premium surcharge for 2015. Why re-attest? – Because UMP Classic’s benefits and premiums are changing in 2015, you must determine whether the spousal premium surcharge will apply using 2015 plan information. You must make your comparison against the UMP Classic plan even if you’re enrolled in a different PEBB medical plan.
To make plan changes or re-attest to the 2015 spousal premium surcharge log into My Account on the PEBB website beginning Nov. 1 or submit a 2015 Premium Surcharge Attestation form to Ellen Pincus, Room 1015. The 2015 Premium Surcharge Attestation form will be available on the PEBB website beginning Nov. 1.
Changes and attestations may be made online Nov. 1 through Nov. 30. If you are submitting a paper enrollment/change form or a paper attestation form, Ellen Pincus in the benefits office must receive the form no later than Nov. 30.
