Dear Colleagues,
As shared previously in messages to our campus community, all College employees (regardless of work modality) must complete the online attestation form regarding their vaccination status, as well provide proof of vaccination by the upcoming October 18, 2021 deadline.
Although I can see that you have completed the online vaccination attestation (thank you!), please ensure that you also provide proof of vaccination to HR to complete the vaccination verification process.
URGENT: The attestation and proof of vaccination to Human Resources must be completed no later than October 18, 2021.
The validation of vaccination documentation will be conducted one-on-one with a Human Resources staff member, either online or in-person. Please be ready to show the following items:
- Photo identification
- Proof of vaccination*
- CDC COVID-19 Vaccination Card or photo of the card; or
- Documentation of vaccination from a health care provider or electronic health card; or
- State immunization record; or
- Employees vaccinated outside the U.S. may show a reasonable equivalent of these documents
The following times are available to briefly connect with Human Resources to show proof of vaccination.
